Volunteer & COVID-19 Guidelines, Assumption of Risk, Waiver of Liability Agreement

Volunteer Guidelines: I acknowledge that I have voluntarily applied to Fostering Greatness Inc.’s volunteer list. I understand that the scope of my volunteer relationship with Fostering Greatness Inc is limited to a volunteer position and that no compensation is expected in return for services provided by me; that Fostering Greatness Inc. will not provide any benefits traditionally associated with employment; By agreeing to volunteer with Fostering Greatness Inc., I agree to hold Fostering Greatness Inc., its entities, its employees, and all associated parties harmless in the result I suffer or sustain personal illness or injury while performing volunteer services.

Signing In and Out: I understand the importance of signing in and out each day of service. In addition, I will complete all information required and make sure to wear my badge during each time of service. I agree to follow all protocol for volunteering with and for Fostering Greatness Inc.

Anti-Harassment Policy:  I understand that Fostering Greatness Inc. is committed to maintaining an environment free from harassment and sexual harassment. Harassment and sexual harassment are forms of discrimination, and harassment because of and discrimination against persons in all protected classes is prohibited. All employees and volunteers are required to work in a manner that prevents harassment and sexual harassment. Fostering Greatness Inc. will supply each volunteer with a copy of the Anti-Harassment policy. Volunteers are expected to read the policy and sign an Acknowledgement that they have received and read the policy, and that they will abide by its guidance.

Dress Code:  I understand that all volunteers need to adhere to all dress code, escort requirements and procedures while volunteering for Fostering Greatness Inc.

Confidentiality:  I understand that in the course of my volunteer service with Fostering Greatness Inc. I may have access to and become acquainted with information of a confidential nature related to the operation of this nonprofit organization, and its clients. Such confidential information includes, but is not limited to, devices, mailing lists, processes, records and information concerning clients. I agree that I will not disclose any of the above mentioned directly or indirectly, or use in any way, either during the term of my volunteerism or any time thereafter.

Photo and Video Release:  I, furthermore, grant permission to Fostering Greatness Inc. to use my photograph or video (alone or in a group) for the purposes of publications, website, promotion, or any other use deem necessary.

I understand that I serve in an at-will capacity; this means that a volunteer can be released from duty at any time without any reason. It also means the volunteer can resign at any time without any reason. Fostering Greatness Inc. is not required to provide notice when terminating a volunteer and that this agreement is not to be construed as constituting a promise of continued volunteer service or employment

COVID-19 Guidelines:

  • Fostering Greatness Inc. requires all volunteers to comply with any and all New York State executive orders as a pertains to Covid 19. This includes travel restrictions, quarantine orders, and testing procedures.

  • I am aware that I must wear a face mask when prohibited.

  • I attest that I am not experiencing any symptoms of illness such as a fever, cough, or shortness of breath. If I develop these symptoms, I agree that I will cancel my shift before arriving at the site to volunteer with Fostering Greatness Inc.

  • I am aware that I must follow the safety and hygiene protocols that have been implemented Fostering Greatness Inc. and that are posted onsite for my review.

  • I attest that:

    • I do not believe that I have been exposed to a person with a confirmed or suspected case of COVID-19

    • I have not been diagnosed with COVID-19 and not yet cleared as noncontagious by state or local public health authorities

    • I am following recommended guidelines as much as possible - practicing social distancing by participating in group activities of fewer than 10, trying to maintain separation of six feet from others, and otherwise limiting my exposure to the coronavirus

Assumption of Risk and Waiver of Liability: I understand that my volunteer activities with Fostering Greatness Inc. may include activities that include but not limited to packing, loading, unloading and carrying heavy items, transportation to and from sites, and exposure to people with lived foster care experiences. I fully understand and appreciate the risks that are inherent to my volunteer activities. I hereby assume the risk of bodily injury, illness, infectious disease, medical treatment resulting from my volunteer activities even if resulting from the negligence of Fostering Greatness Inc. or its employees or clients.

I hereby release, discharge and agree to indemnify and hold Fostering Greatness Inc. harmless from, and waive on behalf of myself, any and all causes of action, claims, demands, damage, costs, expenses and compensation or loss to myself that may be caused by any act, or failure to act of Fostering Greatness Inc., or that may otherwise arise in any way in connection with any voluntary activities with, or for Fostering Greatness Inc..

This liability waiver and release extends to Fostering Greatness Inc. together with all its Board of Directors, affiliates, employees and clients.
